This website provides general educational information and information about SimplyNaya services. It does not provide medical advice, diagnosis, treatment, emergency services or a guarantee of hospice eligibility or admission.

Emergencies

Do not use SimplyNaya for an emergency. Call 911 or the appropriate emergency service.

Professional guidance

Questions about health conditions and treatment should be directed to a qualified healthcare professional. Any hospice relationship is directly between the patient or family and the hospice provider.

Text messaging

When a caller affirmatively requests a directory link, SimplyNaya may send a one-time text message containing the requested information. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. SimplyNaya does not send recurring promotional text messages through this interaction. Consent is not a condition of purchasing healthcare services.

Local searches and provider information

Use the location where care is needed when searching for local support. A suggested location can be edited before you confirm your search. The person looking for information may be in a different city from the person receiving care, so an automatically suggested location may not be the right care location.

Provider information describes reported service areas and available directory information. A listing does not guarantee that an agency currently accepts new patients, serves a particular address or can begin care immediately. Confirm service availability, insurance, costs and timing directly with the provider. SimplyNaya does not choose a hospice for you or determine whether a person qualifies for care.

Private directory links

The provider-directory link sent by text is available for 48 hours. It is intended to help you review local options and prepare questions for an agency. Receiving a link does not enroll anyone in hospice or create a care agreement with a provider. Keep the link private and avoid posting it publicly.

Privacy and safe use

SimplyNaya calls are never recorded. Please do not submit medical records, detailed health histories or other sensitive medical information through website contact or partner forms. Provider partnerships

These website terms describe use of the consumer information service. Agency partnerships, pricing and activation are addressed separately through a provider agreement. Visiting a partner page or asking for information does not itself activate a partnership.
